Black titanium is implant-grade titanium with a black finish applied to the metal surface rather than plated over a base metal. You get the matte black look with titanium underneath — nickel-free, light, and suited to sensitive skin.

As with any dark finish, treat it as a healed-piercing material and avoid abrasive cleaners, which will dull it. If you want black at a lower price and are not sensitive to nickel, black steel is the alternative.
